Who pays the app store fees?

You do, in your own developer accounts. That is deliberate rather than a way of avoiding a cost: an app published under our account would not really be yours, and moving it later is painful. Apple also charges annually to keep an app listed.

How long does app store approval take?

Days usually, longer sometimes, and it is not our decision. Both stores review submissions against their own rules and can come back with changes. We handle the resubmission, but nobody honest will promise you a launch date.
